What you will need
to get started

Step 1

You may contact us via phone, email, or submit a Contact Form online with your information. We will be in touch to discuss scheduling and the services that will best suit your needs.

Step 2

 We will need a physician's referral for all services. You can make a request for this to your child's pediatrician. Referrals may be sent via fax or the patient portal.

Step 3 

Complete an Intake Form and New Client Policy Packet - We will provide you with a link following initial contact to sign up for our Patient Portal Account where you can complete the paperwork needed prior to your child's evaluation.
